Modernizing Medicine, Inc. (ModMed) discovered unauthorized activity in certain computer servers used by its podiatry practice clients. Between July 9 and July 10, 2025, a threat actor accessed and copied sensitive information stored on these internal servers. Personal and protected health information exposed in the data breach includes full name, address, date of birth, phone number, email address, Social Security number, health insurance information, and more. ModMed began notifying impacted practices on September 19 and mailed data breach notices to affected patients on October 17.
